Thank you for your purchase of Annual Membership in the Colorado Country Music Hall of Fame!  This confirms that we have received your payment via PayPal.

It's important that we have current information on file to accompany your membership so that we can send you notices and updates of importance to Country Music fans here in Colorado.

In order to do that, be sure that you have filled out one of the forms listed above or shown here below.  You can fill it out online using the latest versions of Adobe Acrobat Reader and submit to us via email.  Because you paid online, no signature is necessary.

Submitting your information online

Follow these steps to furnish your information online.  If you need the latest version of Adobe Acrobat Reader®, click for more information.

1.  Select the proper PDF form:

• Bands use this form - Band Membership
• All other types of membership use this form - Individual Membership

These forms will open in a new window or tab to allow you to come back to this page for instructions.

2.  Fill out the pertinent sections of the form in the other tab or window.  When completed, save the form.  You can click on a save logo on the PDF window as shown below.  You'll be prompted to name the file (we suggest you name it FirstName.LastName.Membership: john.smith.membership.pdf) and can choose to store it on your desktop or wherever convenient.

3.  Send the form in an email to .  In future years, you can submit the saved PDF again, or even make address or other changes before sending to us as desired.

Adding or Upgrading Adobe Acrobat Reader®

If you do not have Adobe Acrobat® you can click on the logo at left to download the latest version directly from Adobe.

These  Membership PDFs take advantage of the latest Adobe Acrobat PDF® form management.  When you open the form you will be able to type or use the check boxes to fill it out; once completed, follow the instructions above to save, print and forward it to us.
